　　高中起点升专科、本科：应届和往届高中、中专、职高毕业生及同等学历者。

　　专科升本科：已取得或将取得国民教育系列专科文凭者。

　　(一)拥护中国共产党的领导，拥护四项基本原则，遵纪守法，品德良好，身体健康。

　　(二)在职、从业人员，高中、中专、技校应届毕业生及社会其他人员。

　　(三)报考高中起点本、专科考生应具有高中毕业文化程度，报考专科起点本科及第二专业专科的考生必须是已取得经教育部审定核准的国民教育系列高等学校或高等教育自学考试机构颁发的大学专科毕业证书的人员。

　　(四)卫生部办公厅、教育部办公厅、国家中医药管理局办公室发布关于对成人高校医学门类专业考生报名资格审查工作的补充通知。按照《教育部卫生部关于举办高等医学教育的若干意见》的'有关要求，该通知对成人高校医学门类专业招生的考生报名资格审核工作提出以下意见：一、考生报考成人高校医学门类专业除符合《教育部关于做好全国成人高校招生工作的通知》附件中规定的报名条件外，还须符合下列要求：

　　1、报考临床医学、口腔医学、预防医学、中医学专业的人员必须已取得省级卫生行政部门颁发的相应类别的执业助理医师及以上资格证书，或者县级及以上卫生行政部门颁发的乡村医生执业证书并有中专学历或中专水平证书。

　　2、报考护理学专业的人员原则上须取得省级卫生行政部门颁发的护士执业资格证书。

　　3、报考医学门类其他专业的人员必须是在职卫生、医药行业技术人员。

　　4、考生所报考的专业原则上应与所从事的专业对口。

　　成考考试确认报名所需材料

　　1、考生在网报后，前往区县招办确认，须按本人在网上选取的报名确认点和日期办理报名资格审查、摄像手续，签署《考生诚信考试承诺书》等。

　　身份证原件(25岁以上(1981年2月28日前出生)或少数民族考生需要复印件1张)。
